SUMMARY OF OPERATIONS
ELIGIBILITY
Only enrollees (members) of our corporate and individual policies have access to any of the healthcare Providers in our National Network at the local level and internationally too, when the need arises. Our scheme is designed to cover the principal members and / or his dependants (spouse and children) or as many spouses and children as may be allowed, provided there has been an initial Pre-paid-premium.
EMERGENCIES
Under an emergency situation, enrollees are expected to seek emergency care at the nearest hospital in the vicinity. Clearline should be notified immediately after the enrollee has been stabilised. Covered Beneficiaries are advised to choose hospitals nearest to their residences in case of emergency. Treatments outside our provider hospitals network is discouraged unless with authorisation from the HMO.
CHANGE OF PROVIDER
Beneficiaries can change from one provider to another after due notification to HMO or by completing a change of hospital form or by contacting the relationship officer. This can be done only once in a month. Where an enrollee needs his ID card replaced for any reason including loss and change of plan, new HMO ID Cards will be prepared for a token administrative fee of five hundred naira (N500.00).

We deploy a customer-oriented approach in dealing with all our clients by assigning a Medical case officer and a Client service officer to each organisation to take care of the medical and administrative issues pertaining to staff medical service.
We recognise the importance of a critical aspect of the business by providing a 24-hour Customer-care Centre with over 10 mobile lines, manned by medical personnel on call basis to respond to the needs of enrollees.
This is in addition to the Pre-engagement medical screening (for chronic diseases) held before the commencement of scheme. A Wellness Programme is organised for corporate organisations registered on our scheme at an agreed period of the year, during which health education and medical screenings will be carried out on staff. Confidentiality of test results is assured.
We operate a robust, nation-wide secondary provider network, with most of the top notch specialists especially for dental and optical care on board. Paediatricians, Eye and Dental consultants are first line doctors at Clearline HMO.
A periodic utilisation report detailing attendance and disease distribution per location is made available on request to the HR Department of Corporate Organisations.
NEW ENROLMENTS, SUBSTITUTIONS & RENEWALS
New employees of the company shall be registered upon due notification to HMO. Where a Covered Beneficiary has spent up to six (6) months on the scheme, the Covered Beneficiary will be deemed to have used the scheme.
Therefore, in the event that such Covered Beneficiary disengages from the scheme, the Covered Beneficiary cannot be replaced by an incoming beneficiary.
The HR specialist of Corporate organisations should send an updated list of staff/beneficiaries and applicable health plans to HMO, on the basis of which the HMO shall prepare and present an invoice stating premium payable by the company.
ADDITIONS TO FAMILY
Any addition to the family on dependants list should be done by filling an inclusion form with passport photograph. New born babies are covered by mothers I.D card for the first 6 weeks after which Registration must be done.
SEEKING MEDICARE
The HMO ID card issued by Clearline simplifies access to medical service at the hospital. It must always be presented by enrollees before treatment as it protects the Provider against impersonation. It is advisable to choose only one hospital for the family, if they are together or specific hospitals for each member of the family if he/she is in a different location. However any enrollee can use ANY Hospital on our Network of Providers in cases of emergency without necessarily being registered there.
USE OF HOSPITALS OUTSIDE OUR REGULAR NETWORK
Use of ANY hospital in Emergency, to SAVE LIFE is encouraged and allowed. You must inform any of the Call Centers / Help Lines at once (within 48 hrs). Clearline will contact the nearest office anywhere in Nigeria to despatch a Medical Team to assess the situation, authorize continuation of treatment or transport to the most appropriate care center by the fastest and safest means.
ENCOUNTER DATA FORM
Every enrollee is expected to sign a completed form at the hospital after accessing medical care, for each visit to the Provider. It is very important for quality control.
REFERRAL SYSTEM
Enrollees requiring referral to secondary facilities within covered services are referred automatically within the facility; where the facility falls outside the primary provider’s zone, a pre-authorization is obtained from the call centre and the appointment fixed. This is waived in emergencies and paediatrician consults.
